Ohio Home Care
Our waiver programs are provided in conjunction with the Ohio Department of Jobs and Family Services. You must be Medicaid eligible to qualify for these programs. Eligibility requirements vary from regular/community Medicaid. The application process is simple. Ask us how you can request a free in-home assessment.

The Older Americans Act (Title III) is a federally funded program that provides older adults the option to take advantage of in-home and community based services as an alternative to nursing home care.
Information and Referral
Your one stop shop for information on services in programs in our four county service area. The community resource department offers comprehensive information for community programs buts also resources in Northeast Ohio and across the state.
Additional Services & Programs
Our Ombudsman Department offers advocacy for those most vulnerable in our community. Any resident of a Long-Term Care facility, Group Home, or member of any of the waiver service programs has access to help, advice, and mediation services through this program.
Living well does not have to be a premium option. The Area on Aging 11 offers the highest quality of evidence based programs to fit your health needs and lifestyle. Each workshop is free of charge and open to all members of the community. Join us in helping Northeast Ohio become healthier.
