Full time position – Under the direction of the Director of Home and Community Based Services, the Recovery Manager includes coordinating all service received by an individual and assisting the individual in gaining access to needed Medicaid State Plan and 1915(i) services, as well as medical, social, educational, and other resources, regardless of funding source.
Required Education and Experience:
Bachelor’s degree in Social Work, Counseling, Psychology, Registered Nurse, or similar field required. A minimum of three years post degree experience working with individuals with severe and persistent mental illness (SPMI). Training in administering ANSA, person-centered planning, evaluating HCBS living arrangements, 42 CFR part 2 confidentiality of alcohol and drug abuse patient records, incident reporting and meets state conflict of interest standards.
Applicant must demonstrate the knowledge and skills necessary to provide the level care management/assessment appropriate to the scope of program responsibilities.
Applicant must be flexible to 24 hour coverage and access to database when necessary.
Experience with any of the following is desirable: pediatric patients, home care for pediatric patients, mental health patients, or physically disabled patients.
Care Manager/Waiver Service Coordinator
Full time - Under the general supervision of the Director of Home and Community Based Services, the Case Manager conducts face to face meetings with consumers in community settings and develops care plans and the implementation of services.
Required Education and Experience: Registered Nurse or Licensed Social Worker with at least one year prior experience in home health care, medical social work and/or geriatrics; an understanding of Medicare, Medicaid and mental/behavioral health experience is helpful. Local travel required. Occasional on-call rotation may be required. Will work at the Agency’s corporate office with the option of working from home after successful completion of the orientation period.
Full time position – Under the general supervision of the Director of Home and Community Based Services and/or their designee, the Care Coordinator is responsible to know and abide by specific program contractual expectations. The Care Coordinator is expected to manage and coordinate comprehensive services and assure that they are appropriate and meet the needs of the consumer to support them with the highest quality of life possible while remaining safely in a community/home setting of their choice. The Care Coordinator conducts assessments in families’ homes and facilitates needs/services for caregivers in the Mahoning County Levy Program.
Required Education and Experience:
Applicant must be a Registered Nurse or Licensed Social Worker with current, unencumbered license, with at least one year prior experience in home health care, medical social work and/or geriatrics. Demonstrates working knowledge and skills necessary to provide the assessment and care management required by the consumer. Applicant must demonstrate flexibility in schedule and in adapting to rules changes and productivity changes and possesses an understanding of 1.) Medicare, Medicaid and third party reimbursement and 2.) Community service delivery systems and the ability to link consumers to community's resources.
