Programs & Services

If you are looking for services, there are a number of programs that can help. Services available range from those for older adults who are independent to those in need of assistance with their daily activities.

  • The Adult Foster Home Program works with small group homes in the community to become providers for the Residential State Supplement (RSS) program.
  • The Assisted Living Waiver is a Medicaid program which provides financial support to individuals age 18 and over who qualify for Medicaid and would do well in an assisted living facility.
  • Caregiver Support offers support to caregivers caring for someone age 60 and over. Services may include information, education, respite, support groups and care management.
  • Home and Community Based Services are those provided in the home of an individual or in a community setting, such as an adult day care or senior center.
  • Housing options are available to meet a variety of living needs for older individuals.
  • The Long Term Care Ombudsman Program provides advocacy services for older adults using long term care services, whether in a facility or in their own home.
  • Nutrition Services provide meals to individuals age 60 and over, either in a congregate or group setting in the community or through meals delivered to the home of the individual.
  • PASSPORT is the state's Medicaid waiver program for people age 60 and over. The program offers a variety of services, including assessment of need, review prior to nursing home placement, and, for those eligible, in-home care. To be eligible for PASSPORT Home Care, an individual must meet certain financial guidelines as well as being at a certain level of need.
  • Pre-Admission Review
  • Protective Services offer support for older adults at risk of abuse, neglect or exploitation.
  • Public Benefits are programs from which older individuals may be entitled to receive services. These may range from health insurance through Medicare or Medicaid to prescription assistance to energy assistance programs.
  • Senior Centers are community organizations that provide information, education, programming and other services for individuals age 60 and over.
  • Senior Olympics provides amateur athletic competition to men and women age 50 and over. The games presented by Area Agency on Aging 11, Inc. are open to competitors regardless of location.
  • Volunteer Services offer older individuals and other to get involved in their communities in a variety of long term and short term projects.
