Essential functions:

Possess a valid driver’s license and proof of insurance
Physical demands, including strength, climbing or balancing. Ability to ambulate enough to access private homes/locations which may not be readily accessible
Must pass criminal background check and drug screen
Ability to effectively communicate with the adult population (including mental health and mentally retarded/developmental disabled individuals) and the general public, both in person and by phone. Ability to comprehend written material related to essential job functions,
Ability to accurately document facts related to essential job functions,
Ability to comprehend, recall, and apply facts related to essential job functions,
Ability to analyze, evaluate, and implement a reasonable course of action based on available information,
Ability to detect building code violations and specific deficiencies related to the consumer's environment,

Position(s) Available:  Care Manager, Assessor, Case Assistant
 
 
 

Care Manager:

Full time - Under the general supervision of the Director of Home and Community Based Services, a care manager meets face-to-face with consumers in community settings and develops care plans and the implementation of services.

 

Requires person to be a Registered Nurse or Licensed Social Worker with at least one year prior experience in home health care, medical social work and/or geriatrics; an understanding of Medicare, Medicaid and third-party reimbursement is helpful. Person will work from Agency’s corporate office but travel is required.

 

Starting Salary range based on experience: $32,186.00 range to $38,623.00


 

Housing Case Manager
 
Full Time - Under the general supervision of the Chief Operating Officer, the Housing Case Manager is responsible for the assessment, management and coordination of a comprehensive plan for the housing needs relating to minor home modification of consumers in the designated area.
 
Services are to be appropriate and meet the needs of the consumer to support them with their housing needs while maintaining cost effectiveness. Bachelor’s degree or equivalent with prior knowledge and or experience in housing, home repairs is preferred. Applicant must demonstrate working knowledge and skills necessary to work with housing codes, city codes, zoning ordinances universal design as well as serving as project point-person for home repair project.
 
Candidate needs to demonstrate flexibility in schedule and in adapting to rule changes and productivity changes. Person must have a valid Ohio driver's license, proof of insurance and the ability to travel.
 
Salary Range is $32,186-$38,623.
